Some of the adverse effects occurs which may cause life threatening conditions

Before starting the treatment, patient must get advice from physician about demerits of the HepBest tablets which may occur during or after the therapy

One of the major adverse that occurred after discontinuation of HepBest tablet, severe acute aggravation of HBV infection, this may happens due to stoppage of tenofovir Alafenamide which is involved in the treatment of hepatitis B infection

HepBest performed its action by

HepBest is a prodrug; TAF is a phosphonoamidate of tenofovir.

TAF has lipophilic cell impermeability effect, which percolates into primary hepatocytes by undergoing passive diffusion and also by hepatic intake transporters like OATP1B1 & OATP1B3

Inside the cells, TAF is changed into tenofovir by hydrolysis with the help of carboxylesterase present in hepatocytes cells

Again tenofovir is phosphorylated into pharmacologically active metabolite tenofovir diphosphate by cellular kinase

This metabolite involved in the inhibition of viral replication by insertion into viral DNA by reverse transcriptase, concluded in termination of viral DNA chain


Body works for HepBest after administration

The maximum peak concentration level occurs within 0.48hours after an oral administration

The effect of high fat meal corresponding to fasted state occurs in the range of 1.51 hours & 1.81 hours

HepBest tablet is highly bound to human plasma protein within the range of 80%

The blood to plasma ratio is 1.0%

The metabolism of tenofovir Alafenamide is occurred through hepatically with the help of carboxylesterase, cathepsin A & CYP3A

The elimination of HepBest tablets;

<1% of metabolite through urine

31.7% of metabolites through feces

Nearly 80% of drug gets metabolized in liver

The terminal half life period of HepBest tablet is 0.51 hours


Side effects caused during therapy

The most severe adverse effects occurs during or after the therapy are;

Lactic acidosis or severe hepatomegaly with steatosis

Severe intensive worsening of hepatitis B viral infection

New outset or exacerbation of renal impairment

The main common side effects are

Head ache

Abdominal pain




Back pain

Decreased in serum phosphates level

Depletion in bone mineral density


Amylase and lipase level increased



Drug interaction occurred while receiving HepBest

HepBest is a substrate of P-gp & breast cancer resistance protein; P-gp & BCRP strong inhibitors are concomitant with HepBest tablets which may cause altering the TAF absorption

HepBest tablet concurrently used with P-gp inducers leads to decrease the TAF absorption, which concluded in depletion of plasma concentration and causes low therapeutic effect

HepBest co administered with moderate P-gp & BCRP inhibitors causes elevation of absorption and plasma concentration of tenofovir Alafenamide

Mostly tenofovir is excreted via urine, through kidney; combination of HepBest tablet with the drugs which is reducing the renal function which may causes elevation of tenofovir concentration & renally excreted drugs causes some adverse effects

Examples: Acyclovir, valaciclovir, amino glycosides, Ganciclovir, cidofovir, NSAIDS

HepBest tablet concomitant with anti-convulsants drugs like phenytoin, Phenobarbital, carbamazepine causes decreasing the concentration of TAF

HepBest tablets with anti-mycobacterials like rifabutin, rifampin or rifapentine may leads to decrease the TAF

Tenofovir Alafenamide combined with herbal product like st. Johns wort causes decreasing in serum concentration of tenofovir Alafenamide


Reason for withhold the therapy

No contraindication occurred during therapy

Some anaphylactic reactions may occurs if patient are contraindicated to the component present in HepBest tablet


HepBest dosage

Before starting the therapy, patients should be investigate for HIV-1 infection, in case of HIV-1 infection present, tenofovir Alafenamide should not be used alone

Before initiating the treatment patients must be examine the values of

Serum creatinine

Serum phosphorus

Creatinine clearance

Urine glucose

Urine protein

During the therapy, patients are clinically observed and maintain the values of these levels


Recommended dosage:

The prescribed dosage of HepBest tablet is 25mg should be taken as a single dose by administering with food

No dosage adjustment should be done in both renal & hepatic impaired patients



HepBest tablets, the safety and effectiveness of HepBest in pediatric patients less than 18 years of age


How to take HepBest tablets

HepBest tablet is a single dose regimen; it must be taken as once a day

HepBest tablets are administered orally with whole some water, it should not be crushed, chew, do not break the tablet

HepBest tablet should be taken with food

It is a prescription medicine, used only under the guidance of medical practitioner


Precaution takes place during the therapy

After completion of therapy, some adverse effects may occur to overcome the problems some precaution may take place;

Conditions like;

Lactic acidosis or serious hepatomegaly with steatosis:

Obesity and extended nucleoside exposure are the risk factors of this condition; if this adverse effect occurs then the therapy with HepBest should be postponed

After cessation of anti-hepatitis B therapy, severe worsening of hepatitis B viral infection occurs, to prevent this effect patient who are discontinue the therapy should be examine frequently to overcome these problem and continue the treatment

Serious risk of advancement of HIV-1 resistance in patients who are co infected with HBV & HIV-1: To control this effect, patients who are infected with hepatitis B infection should be monitored with HIV antibody testing before starting the treatment. Anti-retroviral drugs combination also suggested for co infected patients

Aggravation of renal impairment occurs; check Serum creatinine, Serum phosphorus, Creatinine clearance, Urine glucose, Urine protein frequently to avoid these problems, otherwise stop the treatment using with HepBest to escape from renal related adverse effects



HepBest tablets container should be stored at room temperature at below 30oC (86oF)

HepBest container should be keep away from moisture, light & heat

HepBest tablet is safe in use

HepBest tablet used in pregnancy condition only after getting advice from medical practitioner

In some group of patients, placental transformation occurs, in such condition HepBest not recommended

Missed dosage

HepBest is a prescription medicine, if patients fail to take the dose must be consult with trained physician and take the dose within the time; otherwise the missed dose should be skipped and follow the regular dosing schedule

Over dosage

Generally, HepBest tablet is a single tablet dose. If over dosage is occur, first patients must examine with toxicity level and secondly hemodialysis takes place

Hemodialysis is involved in removing tenofovir (unchanged form) from the blood with the eradicating range of 54%
